About Your Visit & Office Policy

What should you bring to your appointment with Orthopedic Center of Palm Beach County

In order to make your visit as efficient and thorough as possible, please bring the following items:

Updated Health Insurance Card/Information
Government-issued Photo Identification
Social Security Card – Medicare Patients only
If needed, a friend or family member to take notes/help with questions
A list of all medications you are taking
A list of your questions /concerns
Copy of your XRAYS / CT/ MRI scans (not just the reports - please get the actual films as well)
Copies of records from doctors related to your visit with us
Current Referral form if required by your insurance
